Applications are accepted primarily from county Department of Social Services and other recognized social agencies.

Children considered for admission must be:
- Boys and Girls age 9-21.
- Have IQ of 70 or above.
|

CAN'T CONSIDER:
Severely emotionally and/or mentally disturbed children and children unable to perform the basic functions of daily living, children dangerous to themselves or others, "Willie M." classification, chronic runaway behavior.
Most referrals are for Foster Care children in the custody of any County Department of Social Services in North Carolina and the NC Department of Juvenile Justice and Delinquency Prevention from neighboring counties.
Application Form: Uniform "Application for Service." The following documentation is required with application:
- Complete Social History
- Medical History
- Psychological (within one year)
- Immunization Record
- Copy of most recent court order
- Birth Certificate
- Medical Exam
- School Records (exceptions made as necessary for emergency care applications)
Application Procedure: Application is official when form is completed and signed by legal custodian. Decision within 10 days after application is received and pre-admission visit is made, as appropriate. Consent and release forms are necessary. Service Plan will be completed upon admission.
Financial Expense Agreement:
- 1. Moderate Group Care: $45.00 per day plus a monthly personal allowance and/or any higher standard foster care board rate set by the state office of DHHS.
Clothing Allowance: $45.00 per month and $150.00 initial allowance for children without clothing upon admission.
Length of Care: No maximum
- 2. Emergency Care: $45.00 per day plus a monthly personal allowance and/or any higher specialized service rate set by the state office of DHHS.
Clothing Allowance: $45.00 per month and $150.00 mandatory initial allowance for children without clothing upon admission.
Length of Care: 90 days
- 3. Respite Care: This program provides delinquency prevention services sponsored by the North Central Alliance for Youth in the Kerr-Tar Regional Council of Goverments (Person, Caswell, Granville, Vance, Frankin and Warren counties).
- Parents are responsible for clothing and medical expenses.
- Length of Care: 30 days
